Job Requirements
- Current RN license or eligibility to obtain
- Minimum 1 year nursing experience
- Compassionate and dependable attitude
- Valid driver’s license and reliable transportation
- CPR certification
- Ability to work independently during nights, weekends, holidays
- Meet state specific requirements if applicable
Job Qualifications
- Current RN license in the state of employment or eligibility to obtain
- 1+ years of nursing experience
- Compassionate, calm, and dependable in urgent care situations
- Excellent clinical judgment
- Comfortable providing care in patients’ homes, facilities, or other residences
- Valid driver’s license, automobile insurance, and reliable transportation
- Current CPR certification
- Preferred: 1+ years of hospice, home health, oncology or palliative care experience
- Familiarity with symptom management, triage, and after-hours care
- Experience in on-call or overnight nursing roles
- Strong documentation and EMR skills
Job Duties
- Provide direct patient care and support after normal business hours
- Serve as the primary liaison between patients, families, physicians, and hospice team during shifts
- Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
- Evaluate patient and family needs to guide clinical decisions and implement care plans
- Collaborate with interdisciplinary staff including LPNs/LVNs, CNAs, and support staff
- Accurately document clinical visits and communications
- Participate in quality assessment and performance improvement programs
