Job Requirements
- active RN license in the state of employment or eligible to obtain
- one or more years of general nursing experience
- valid driver's license, auto insurance, and reliable transportation
- current CPR certification
- comfortable visiting patients in various healthcare settings
- ability to complete required documentation and comply with state and federal regulations

- strong assessment, communication, and interpersonal skills
- comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
- valid driver's license, auto insurance, and reliable transportation
- current CPR certification
- two or more years of hospice, home health, healthcare admissions, or medical sales experience preferred
- Bachelor's degree and/or Certified Hospice and Palliative Nurse (CHPN) certification preferred
- background in case management, care coordination, or healthcare outreach preferred
- experience with electronic medical records and compliance documentation preferred
Job Duties
- visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
- explain hospice philosophy and services clearly, compassionately, and with clinical confidence
- guide patients and their loved ones through the admissions process
- collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
- use the nursing process to assess patient needs, support care planning, and document clinical findings
- build trust and provide emotional reassurance to patients and families
- accurately complete required documentation and ensure compliance with state and federal regulations
- work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
