Job Requirements
- active RN license in the state of employment or eligible to obtain
- 1+ year of general nursing experience
- current CPR certification
- valid driver’s license, auto insurance, and reliable transportation
- ability to visit patients in hospitals, skilled nursing facilities, or home settings
- ability to meet state-specific licensure and regulatory requirements
- commitment to a drug-free workplace
- ability to work in a safety-sensitive position
Job Qualifications

- 1+ year of general nursing experience
- a hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
- strong assessment, communication, and interpersonal skills
- comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
- valid driver’s license, auto insurance, and reliable transportation
- current CPR certification
- 2+ years of hospice, home health, healthcare admissions, or medical sales experience preferred
- bachelor’s degree and/or certified hospice and palliative nurse certification preferred
- background in case management, care coordination, or healthcare outreach preferred
- experience with electronic medical records and compliance documentation preferred
Job Duties
- report directly to the admissions manager and serve as a key ambassador for hospice services
- visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
- explain hospice philosophy and services clearly, compassionately, and with clinical confidence
- guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
- collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
- use the nursing process to assess patient needs, support care planning, and document clinical findings
- build trust and provide emotional reassurance to patients and families as they make this important decision
- accurately complete required documentation and ensure compliance with state and federal regulations
- work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
