Job Requirements
- Active RN license in the state of employment or eligible to obtain
- 1 plus year of general nursing experience
- valid driver’s license, auto insurance, and reliable transportation
- current CPR certification
- ability to visit patients in hospitals, skilled nursing facilities, or home settings
- meet state-specific licensure and certification requirements
- must be able to complete required documentation accurately and comply with state and federal regulations
- must be a compassionate and empathetic individual committed to patient-centered care

- preferred experience includes 2 plus years of hospice, home health, healthcare admissions, or medical sales
- bachelor’s degree and/or certified hospice and palliative nurse (CHPN) certification
- background in case management, care coordination, or healthcare outreach
- experience with electronic medical records and compliance documentation
Job Duties
- Report directly to the Admissions Manager and serve as a key ambassador for hospice services
- visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
- explain hospice philosophy and services clearly, compassionately, and with clinical confidence
- guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
- collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
- use the nursing process to assess patient needs, support care planning, and document clinical findings
- build trust and provide emotional reassurance to patients and families as they make this important decision
- accurately complete required documentation and ensure compliance with state and federal regulations
- work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
