Job Requirements

  • Active RN license in the state of employment or eligible to obtain
  • 1 plus year of general nursing experience
  • valid driver’s license, auto insurance, and reliable transportation
  • current CPR certification
  • ability to visit patients in hospitals, skilled nursing facilities, or home settings
  • meet state-specific licensure and certification requirements
  • must be able to complete required documentation accurately and comply with state and federal regulations
  • must be a compassionate and empathetic individual committed to patient-centered care

Job Qualifications

  • Active RN license in the state of employment or eligible to obtain
  • 1 plus year of general nursing experience
  • compassionate, empathetic, and deeply committed to patient-centered care
  • strong assessment, communication, and interpersonal skills
  • comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
  • valid driver’s license, auto insurance, and reliable transportation
  • current CPR certification
  • preferred experience includes 2 plus years of hospice, home health, healthcare admissions, or medical sales
  • bachelor’s degree and/or certified hospice and palliative nurse (CHPN) certification
  • background in case management, care coordination, or healthcare outreach
  • experience with electronic medical records and compliance documentation

Job Duties

  • Report directly to the Admissions Manager and serve as a key ambassador for hospice services
  • visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
  • explain hospice philosophy and services clearly, compassionately, and with clinical confidence
  • guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
  • collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
  • use the nursing process to assess patient needs, support care planning, and document clinical findings
  • build trust and provide emotional reassurance to patients and families as they make this important decision
  • accurately complete required documentation and ensure compliance with state and federal regulations
  • work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
