Job Requirements

  • Active RN license in the state of employment or eligible to obtain
  • 1+ year of general nursing experience
  • Valid driver's license, auto insurance, and reliable transportation
  • Current CPR certification
  • Ability to visit patients in home, hospital, or facility settings
  • Compliance with state-specific requirements where applicable

Job Qualifications

  • Active RN license in the state of employment or eligible to obtain
  • 1+ year of general nursing experience
  • Compassionate, empathetic, and deeply committed to patient-centered care
  • Strong assessment, communication, and interpersonal skills
  • Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
  • Valid driver's license, auto insurance, and reliable transportation
  • Current CPR certification
  • Preferred experience includes 2+ years of hospice, home health, healthcare admissions, or medical sales
  • Bachelor's Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
  • Background in case management, care coordination, or healthcare outreach
  • Experience with electronic medical records and compliance documentation

Job Duties

  • Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
  • Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
  • Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
  • Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
  • Use the nursing process to assess patient needs, support care planning, and document clinical findings
  • Build trust and provide emotional reassurance to patients and families as they make this important decision
  • Accurately complete required documentation and ensure compliance with state and federal regulations
  • Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
