Job Requirements

  • Valid RN license or eligibility to obtain
  • Minimum one year nursing experience in hospice or related field
  • Demonstrated compassionate and patient-centered care
  • Ability to provide care in varied settings including homes and facilities
  • Strong interpersonal and critical thinking skills
  • Valid driver’s license and reliable transportation
  • CPR certification

Job Qualifications

  • Active RN license in the state of employment or eligible to obtain
  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
  • Compassionate, empathetic, and patient-centered approach
  • Comfortable providing care in diverse settings including private homes and facilities
  • Strong communication and critical thinking skills in emotionally sensitive situations
  • Valid driver’s license, auto insurance, and reliable transportation
  • CPR certification required

Job Duties

  • Be the primary point of contact for patients and families, guiding them through their hospice journey
  • Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care in their home or assisted living
  • Perform initial and ongoing patient assessments to monitor condition and adjust care as necessary
  • Collaborate with an interdisciplinary team to build and refine personalized care plans
  • Provide emotional, physical, and spiritual support to patients and their loved ones, including education and bereavement guidance
  • Teach caregivers how to safely and confidently care for their loved one, offering practical and emotional support
  • Communicate important patient condition updates to the care team to ensure timely interventions
  • Maintain accurate clinical documentation and adhere to infection control and compliance standards
  • Support quality improvement and scheduling initiatives to ensure consistent and efficient care
  • Help patients and families understand medication administration, hospice philosophy, symptom and pain management, and end-of-life processes
