Job Requirements
- Valid RN license or eligibility to obtain
- Minimum one year nursing experience in hospice or related field
- Demonstrated compassionate and patient-centered care
- Ability to provide care in varied settings including homes and facilities
- Strong interpersonal and critical thinking skills
- Valid driver’s license and reliable transportation
- CPR certification
Job Qualifications
- Active RN license in the state of employment or eligible to obtain
- 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
- Compassionate, empathetic, and patient-centered approach
- Comfortable providing care in diverse settings including private homes and facilities
- Strong communication and critical thinking skills in emotionally sensitive situations
- Valid driver’s license, auto insurance, and reliable transportation
- CPR certification required
Job Duties
- Be the primary point of contact for patients and families, guiding them through their hospice journey
- Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care in their home or assisted living
- Perform initial and ongoing patient assessments to monitor condition and adjust care as necessary
- Collaborate with an interdisciplinary team to build and refine personalized care plans
- Provide emotional, physical, and spiritual support to patients and their loved ones, including education and bereavement guidance
- Teach caregivers how to safely and confidently care for their loved one, offering practical and emotional support
- Communicate important patient condition updates to the care team to ensure timely interventions
- Maintain accurate clinical documentation and adhere to infection control and compliance standards
- Support quality improvement and scheduling initiatives to ensure consistent and efficient care
- Help patients and families understand medication administration, hospice philosophy, symptom and pain management, and end-of-life processes
