Job Requirements
- Active RN license in the state of employment or eligible to obtain
- Minimum of 1 year of nursing experience in relevant fields such as hospice, home health, ICU, oncology, or geriatrics
- CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to provide care in diverse settings including private homes and assisted living facilities
- Strong communication and critical thinking skills especially in emotionally sensitive situations
- Compliance with infection control and documentation standards
Job Qualifications

- At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
- Compassionate, empathetic, and patient-centered approach
- Comfortable providing care in diverse settings including private homes and facilities
- Strong communication and critical thinking skills in emotionally sensitive situations
- Valid driver’s license, auto insurance, and reliable transportation
- CPR certification required
- Experience in hospital nursing (med-surg, ICU, ER, PACU, telemetry, cardiac) preferred
- Experience in admissions, wound care, float pool, or travel nursing preferred
- Experience providing care for terminally ill or seriously ill patients preferred
Job Duties
- Be the primary point of contact for patients and families, guiding them through every step of their hospice journey
- Deliver and document skilled, hands-on nursing care based on each patient’s individualized plan of care in their home or assisted living facilities
- Perform initial and ongoing assessments to monitor patient condition and adjust care plans as needed
- Collaborate with interdisciplinary team members including physicians, LPNs, CNAs, social workers, and chaplains to develop and refine personalized care plans
- Provide emotional, physical, and spiritual support to patients and their families, including education, comfort, and bereavement guidance
- Teach caregivers how to care for their loved ones safely and confidently by providing practical training and emotional reassurance
- Communicate important updates in patient condition to the care team to ensure timely interventions
- Maintain accurate clinical documentation and adhere to infection control and compliance standards
- Support organizational quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care
- Help patients and families understand and navigate medication administration, hospice philosophy, symptom and pain management, and end-of-life processes and expectations
