Job Duties
- Provide culturally proficient education and support to patients eligible for cancer screening
- Ensure understanding of steps needed to complete cancer screening process
- Recruit potential cancer screening participants from priority populations through mailings, phone contact, and face-to-face contact
- Perform case management for abnormal screening results and monitor timely and appropriate specialty care services
- Work with community resources to help patients access support systems and navigate the healthcare system
- Participate in outreach events, track participants, tests performed, and results
- Report outcome information to patients, physicians, and leadership
- Maintain up-to-date information systems for billing and case evaluations
- Perform quality audits and monthly statistics reporting
- Address barriers to successful outcomes of events
- Perform clerical duties and participate in community outreach events
- Distribute and instruct use of FIT kits
- Follow up on returns via phone calls to ensure 90 percent compliance with returns
