Job Requirements
- Must be able to travel throughout Pima County
- High school diploma or GED required
- 2+ years experience in healthcare-related fields required
- Experience with Microsoft Office and general office equipment
- Experience with electronic health records required
- Valid driver’s license and safe driving record
- Reliable transportation
- Ability to work independently and as part of a team
- Ability to maintain confidentiality of patient records
- Physical ability to perform office-related tasks including occasional lifting up to 25 pounds
Job Qualifications
- High school diploma or GED required
- Associate degree in related field or equivalent experience preferred
- Minimum 2 years experience in healthcare-related fields
- Experience in clinical healthcare, managed care, or health plan settings preferred
- Proficient with Microsoft Word, Excel, PowerPoint, Outlook, and office equipment
- Experienced with electronic health records
- Medical Assistant Certification or experience as Pharmacy Tech, Radiology Tech, Lab Tech or similar preferred
- Knowledge of medical terminology
- Experience with CPT II codes preferred
- Current CPR certification preferred
Job Duties
- Build and maintain effective relationships with assigned affiliate groups to achieve goals and key initiatives
- Serve as a liaison between affiliate groups and P3 local, regional, and national departments to provide seamless support
- Advise and support affiliate practices with quality gaps and conditional documentation through outreach, chart data mining, audits, and claims surveillance
- Work with affiliate practices to prioritize patient outreach and assist with scheduling patient visits
- Partner with assigned groups and other departments to manage high-risk patients by coordinating care across healthcare services
- Assist in meeting preparation and participate in CSR/IDT/JOC meetings as requested
- Conduct data entry and maintain documentation and tracking for quality initiatives
- Educate patients and affiliates on preventative screenings, their scheduling, and purpose
- Conduct in-home HEDIS gap closure tasks depending on education and experience
- Perform other duties as assigned
