Job Requirements

  • high school diploma or GED required
  • 2 or more years experience in healthcare-related fields
  • ability to travel within assigned region
  • valid driver’s license with safe driving record
  • reliable transportation
  • proficiency with Microsoft Office and office equipment
  • experience with electronic health records
  • dedication to patient confidentiality
  • ability to handle fast-paced environment and prioritize tasks
  • excellent interpersonal and communication skills
  • strong organizational and time management skills
  • current CPR certification preferred

Job Qualifications

  • high school diploma or GED required, associate degree in related field or equivalent experience preferred
  • 2 or more years experience in healthcare-related fields required, clinical healthcare experience, managed care, or healthplan experience preferred
  • proficiency with Microsoft Word, Excel, PowerPoint, Outlook, and general office equipment
  • experience with electronic health records
  • medical assistant certification or experience as pharmacy tech, radiology tech, lab tech or similar preferred
  • experience with data mining preferred
  • knowledge of medical terminology required, experience with CPT II codes preferred
  • current CPR certification preferred

Job Duties

  • build and maintain effective relationships with assigned affiliate groups to achieve goals and key initiatives
  • serve as a liaison between affiliate groups and P3 local, regional, and national departments
  • advise and support affiliate practices with quality gap closures including outreach, chart data mining, audits, claims surveillance, and educating practice employees
  • assist affiliate practices in prioritizing patient outreach and coordinate scheduling of patient visits
  • partner with assigned groups and P3 departments to manage high-risk patients by ensuring coordination of care
  • assist in CSR, IDT, and JOC meeting preparation and participate as needed
  • conduct data entry and maintain documentation for quality initiatives
  • educate patients and affiliates on preventive screenings and their importance
  • conduct in-home HEDIS gap closure tasks such as blood pressure monitoring and diabetic eye exams depending on experience
  • perform other duties as assigned
