Job Requirements

  • Current RN licensure in the state of employment or multistate Nursing Licensure Compact (NLC) license
  • High school diploma or equivalent
  • Ability to work flexible shifts including on-call and weekends
  • Reliable transportation and valid driver's license
  • Completion of required state and federal clearances
  • Commitment to compliance with organizational policies and regulatory standards

Job Qualifications

  • Zero to two years of nursing experience preferred
  • Six months of nursing experience in an acute care, skilled care, home health, or hospice setting preferred
  • BSN preferred
  • Automotive insurance
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification
  • Valid driver's license
  • Registered Nurse (RN) or Temporary Practice Permit (TPP)
  • Compliance with Act 33, Act 34, and OAPSA requirements

Job Duties

  • Establish patients' plans of care in collaboration with physicians, patients, and utilization management professionals
  • Coordinate case management activities across a multi-disciplinary health care team
  • Initiate, deliver, and evaluate patient- and family-centered health teaching
  • Strive to help patients achieve optimal health outcomes while delivering compassionate care
  • Complete all required documentation including visit assessments, OASIS, 485 and verbal orders accurately and promptly
  • Attend compliance training and adhere to organizational standards, policies, procedures, and regulatory guidelines
