Job Requirements
- Active, unrestricted professional license
- Bachelor’s degree required
- Minimum 2-5 years of care management or geriatrics experience
- Strong knowledge of healthcare systems and care coordination
- Experience working with families in crisis
- Excellent communication and organizational skills
- Valid Ohio driver’s license and current automobile insurance
- Ability to work independently with minimal supervision
Job Qualifications
- Active, unrestricted professional license (LSW, LISW, or equivalent)
- Bachelor’s degree required, Master’s degree preferred
- Minimum 2-5 years of care management or geriatrics experience
- Strong knowledge of healthcare systems, community resources and care coordination
- Demonstrated competence and comfort working with families in crisis
- Excellent interpersonal communication and organizational skills
- Ability to work independently with minimal supervision
- Valid Ohio driver’s license and proof of current automobile insurance
Job Duties
- Conduct comprehensive client assessments including medical, psychosocial, functional and environmental factors
- Develop individualized care plans in collaboration with clients and families
- Maintain client records and complete all necessary documentation including clinical and billing documentation in a timely manner
- Maintain regular in-person, phone and written contact with clients, family/caregivers and other parties to coordinate services and monitor client status
- Coordinate care across providers, community resources and support services
- Provide guidance and advocacy during healthcare transitions and crises
- Facilitate family communication, updates and care conferences and provide education and coaching to clients and caregivers
- Provide a high level of customer service and coordinate individual availability to clients and their families
- Serve as primary point of contact for assigned concierge clients and families
