Job Requirements
- High school diploma or equivalent
- Minimum 1 years of experience in a primary care setting
- Basic Life Support Certification
- Community Health Worker certification required or obtained within 12 months of employment
- Ability to work Monday through Friday
- Strong communication skills
- Experience with case management and patient education
Job Qualifications
- Minimum 1 years of experience in a primary care setting with 3+ years preferred
- Medical Assistant experience is highly desirable
- High school diploma or equivalent, bachelor's degree preferred
- Basic Life Support Certification, Community Health Worker (CHW) certification required or obtained within 12 months of employment
Job Duties
- Manage a caseload of primary care patients by performing initial and regular reassessment of patient needs and developing, implementing, and updating a plan of care to address identified needs
- Resolve patient barriers to accessing health care services by linking with transportation and translation services, appointment scheduling, communication with medical and behavioral health care teams, etc.
- Ensure financial support to facilitate patient access to services including identify and address gaps in insurance status, process sliding scale applications, process applications to linkage to medication programs, and connect patients to available community resources to assist in bridging financial barriers to care
- Provide walk in support to patient's family members, and community members with a focus on quickly decreasing patient obstacles to care
- Provide patient education, guidance, and emotional support to help patient meet identified care goals
- Develop trust and maintain positive relationships with patients, community members, social service organizations and internal SCC teams
- Maintain comprehensive patient records in SCC systems and the patient Electronic Medical Record, ensuring compliance to all confidentiality and HIPAA regulations
