Job Requirements
- Medical degree from an accredited institution
- active medical license to practice in Pennsylvania
- board certification or eligibility in Internal Medicine or Family Medicine
- bilingual proficiency in Spanish
- minimum of two years clinical experience preferred
- experience with outpatient primary care
- commitment to patient-centered care
- ability to operate in value-based care models
Job Qualifications
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- bilingual in Spanish
- fellowship training in geriatrics preferred
- experience treating geriatric populations preferred
- experience in value-based healthcare
- experience in outpatient primary care
Job Duties
- Provide medical care to older adults in a primary care setting
- conduct comprehensive geriatric assessments
- manage chronic diseases and coordinate care plans
- operate within a value-based healthcare model
- communicate effectively in Spanish with patients and families
- collaborate with interdisciplinary healthcare teams
- potentially oversee clinical operations as Center Medical Director
