Job Requirements

  • Minimum Master's degree in Physician Assistant Studies from an accredited program
  • 3-5 years physician assistant experience, no new graduates
  • Prior healthcare experience of 2-3 years in related healthcare roles
  • Active, unencumbered Physician Assistant license in West Virginia and residency in state
  • Current board certification from NCCPA
  • BLS certified with active NPI number as a PA
  • CPR certification
  • Active, unencumbered DEA licensure
  • Preferred experience in home care, primary care, hospice, palliative care, or geriatrics
  • Additional preferred experience with telehealth care delivery
  • Valid driver's license, car insurance, access to a car, and willingness to drive
  • Requires home visits to patients in various residential settings
  • Proof of active auto insurance policy
  • Ability to conduct in-home visits and operate telehealth platforms
  • Proficiency with office software including EHR and scheduling tools
  • Ability to lift up to 30 lbs and assist with transferring patients up to 50 lbs or more
  • Ability to stand for extended periods
  • Ability to drive locally up to 100%
  • Ability to maintain fine motor skills and visual acuity

Job Qualifications

  • Master's degree in Physician Assistant Studies from an accredited program
  • 3-5 years of physician assistant experience
  • Prior healthcare experience in medical assistant, certified nursing assistant, EMT/paramedic, lab assistant, registered nurse, or similar roles
  • Active and unencumbered Physician Assistant license in West Virginia
  • Current board certification with NCCPA
  • BLS certification and active NPI number as a PA
  • CPR certification
  • Active and unencumbered DEA licensure
  • Experience in home care, primary care, hospice, palliative care, or geriatrics preferred
  • Experience with telehealth care delivery preferred
  • Valid driver's license and access to a car
  • Effective verbal, written, presentation, and interpersonal communication skills
  • Proficiency with Microsoft Office, EHR, scheduling software, and telehealth platforms

Job Duties

  • Practice the WellBe mission to help patients lead healthier, meaningful lives by delivering comprehensive care
  • Conduct primarily in-home patient visits to support care needs
  • Partner with other clinicians to manage health and wellbeing of homebound patients
  • Engage attributed patients into the WellBe program and maintain patient engagement
  • Facilitate decision-making by providing educational resources to staff, patients, and families
  • Perform initial patient screenings and physical examinations, create assessments and plans, and complete documentation
  • Address urgent care needs including transition of care visits
  • Collaborate with community teams to complete full patient care assessments and plans
  • Order, interpret, and analyze diagnostic tests and determine follow-up and treatment per state supervisory agreements
  • Prescribe and manage medications and treatments including psychopharmacological agents per state law
  • Update and verify accuracy of patient records at each appointment
  • Deliver patient care and conduct regular patient reviews with supervising physicians
  • Maintain high quality care standards while meeting company patient care metrics
  • Focus clinical care to support documentation, coding, HEDIS measures, patient engagement, and cost management
  • Serve as key resource for patients requiring ongoing care information, counseling, and guidance
  • Utilize internal and external resources to meet patient needs, including behavioral health protocols
  • Collaborate with interdisciplinary team and participate in team meetings
  • Maintain active state licensure and national certification
  • Deliver palliative and end-of-life care and facilitate family meetings to address complex dynamics
  • Adhere to professional conduct standards and complete required documentation
  • Perform other tasks as necessary to meet team goals
