Job Requirements

  • Active PT license
  • BLS CPR certification
  • Comfort with basic technology such as email, texting, tablets, and EMR systems
  • Reliable transportation to visit patients in their homes
  • Ability to work independently and manage time effectively

Job Qualifications

  • Active physical therapist license
  • BLS CPR certification
  • Experience working with geriatric or adult patients in physical therapy
  • Familiarity with rehabilitation and therapeutic exercise techniques
  • Strong communication and patient education skills

Job Duties

  • Deliver in-home physical therapy services
  • Evaluate patient mobility, strength, and function to develop treatment goals
  • Implement effective rehabilitation and therapeutic exercise programs for adult and geriatric patients
  • Educate patients and families on recovery plans and preventive care
  • Maintain accurate and timely home health care documentation
