Job Requirements
- Bachelor’s degree or higher in physical therapy
- 2+ years of general physical therapy experience preferred
- Home health experience preferred but not required
- Current Vermont Physical Therapist license or willingness to obtain one
- Ability to perform comprehensive patient evaluations
- Excellent documentation and communication skills
- Ability to travel within Chittenden and Grand Isle Counties
Job Qualifications
- Bachelor’s degree or higher in physical therapy
- 2+ years of general physical therapy experience preferred or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
- Home health experience preferred but not required
- Current Vermont Physical Therapist license or willingness to obtain one
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Job Duties
- Perform comprehensive evaluations and assessments
- Develop individualized treatment plans tailored to each patient’s needs
- Deliver skilled therapy interventions to improve mobility, strength, and function
- Support patients in remaining in their homes safely and independently
- Collaborate with interdisciplinary team members to provide comprehensive care
- Educate patients and families on therapy techniques and safety
- Document patient progress and maintain accurate records
