Job Requirements

  • Bachelor’s degree or higher in physical therapy
  • 2+ years of general physical therapy experience preferred
  • Home health experience preferred but not required
  • Current Vermont Physical Therapist license or willingness to obtain one
  • Ability to perform comprehensive patient evaluations
  • Excellent documentation and communication skills
  • Ability to travel within Chittenden and Grand Isle Counties

Job Qualifications

  • Bachelor’s degree or higher in physical therapy
  • 2+ years of general physical therapy experience preferred or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
  • Home health experience preferred but not required
  • Current Vermont Physical Therapist license or willingness to obtain one
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team

Job Duties

  • Perform comprehensive evaluations and assessments
  • Develop individualized treatment plans tailored to each patient’s needs
  • Deliver skilled therapy interventions to improve mobility, strength, and function
  • Support patients in remaining in their homes safely and independently
  • Collaborate with interdisciplinary team members to provide comprehensive care
  • Educate patients and families on therapy techniques and safety
  • Document patient progress and maintain accurate records
