Job Requirements

  • High school diploma or general education degree (GED)
  • One to three months related experience and/or training
  • Knowledge of medical terminology
  • Computer skills
  • Knowledge of Empath Health/Suncoast Hospice programs
  • Basic knowledge of reimbursement for services
  • Working knowledge of community resources
  • Ability to listen, assess needs, and coordinate care
  • Possess appropriate license and vehicle for travel needed to perform job responsibilities

Job Qualifications

  • Knowledge of medical terminology

Job Duties

  • Serve as the first responder and primary contact for patients, families, and authorized representatives
  • Ensure optimal access to hospice services and programs
  • Maintain patient-centered services and exceed customer expectations
  • Coordinate care by assessing patient and family needs
  • Provide clear communication and education about available services
  • Utilize knowledge of medical terminology and reimbursement processes
  • Collaborate with community resources to support patient care
