Job Requirements
- High school diploma or general education degree (GED)
- One to three months related experience and/or training
- Knowledge of medical terminology
- Computer skills
- Knowledge of Empath Health/Suncoast Hospice programs
- Basic knowledge of reimbursement for services
- Working knowledge of community resources
- Ability to listen, assess needs, and coordinate care
- Possess appropriate license and vehicle for travel needed to perform job responsibilities
Job Qualifications

Job Duties
- Serve as the first responder and primary contact for patients, families, and authorized representatives
- Ensure optimal access to hospice services and programs
- Maintain patient-centered services and exceed customer expectations
- Coordinate care by assessing patient and family needs
- Provide clear communication and education about available services
- Utilize knowledge of medical terminology and reimbursement processes
- Collaborate with community resources to support patient care
