Job Requirements

  • High school diploma or general education degree (GED)
  • One to three months related experience and/or training or equivalent combination of education and experience
  • Excellent verbal and written communication skills
  • Knowledge of medical terminology
  • Proficiency in computer skills
  • Knowledge of Empath Health/Suncoast Hospice programs
  • Basic knowledge of reimbursement for services
  • Working knowledge of community resources
  • Ability to listen, assess needs, and coordinate care
  • Possess appropriate license and vehicle for travel needed to perform the job responsibilities

  • Serve as first responder and primary contact for patient, family, authorized representatives or customers
  • Ensure patient-centered services and care are prioritized, exceeding customer expectations
  • Provide clear communication and coordinate care needs effectively
  • Demonstrate thorough knowledge of Empath Health and Suncoast Hospice programs
  • Utilize medical terminology and community resource knowledge to assist patients
  • Listen to and assess needs, facilitating appropriate service access and reimbursement guidance
  • Maintain professional conduct and comply with privacy and safety standards
