Job Requirements
- High school graduate or GED
- Valid LPN license
- Minimum 2 years of patient-facing healthcare experience
- Current BLS card for healthcare providers or willingness to obtain
- Ability to document in EMR systems
- Strong geriatric customer service skills
- Strong computer skills with proficiency in Word, Excel, and PowerPoint
- Ability to communicate effectively both verbally and in writing
- Ability to travel locally visiting patient homes
- Ability to lift up to 20 lbs and assist with patient transfers up to 50 lbs or more
- Ability to stand for extended periods
- Must pass pre-employment drug screening
- Comply with drug-free workplace policies
- Must perform duties with full cognitive and physical function at all times
- Adhere to safety-sensitive workplace requirements
- No use of marijuana due to federal safety policies
Job Qualifications
- High school graduate or GED
- LPN license
- Minimum 2 years of patient-facing experience in healthcare practice or hospital
- Current BLS certification or willingness to obtain
- Experience with EMR documentation preferred
- Strong professionalism and customer service skills focused on geriatric patients
- Proficient computer skills including Word, Excel, and PowerPoint
- Excellent verbal, written, presentation, and interpersonal communication skills
Job Duties
- Develop rapport with patients and deliver exceptional in-home services
- Coordinate care with medical team and schedule patient and specialist appointments
- Manage patient medical records and ensure accurate EMR documentation
- Assist with HEDIS gap closure and support quality improvement initiatives
- Act as a community resource and engage unengaged patients
- Conduct non-invasive patient screenings and explain point-of-care testing
- Collaborate in interdisciplinary team meetings and escalate clinical concerns appropriately
