Job Requirements

  • must possess a high school diploma or General Educational Development (GED) certificate
  • be at least 21 years old
  • minimum 1-year experience transporting individuals with special mobility needs
  • interact in a friendly and professional manner with participants and their families
  • communicate effectively and openly with other Interdisciplinary Team members on patient status and needs
  • only act within the scope of his/her authority to practice
  • meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by regulatory agencies before working independently
  • be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
  • pass a criminal background which will verify that the candidate is not excluded from participation in Medicare and Medicaid programs
  • has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act
  • and have not been convicted of physical, sexual, drug or alcohol abuse
  • valid CA driver’s license and an acceptable driving record
  • upon employment, a defensive driving course, first aid, and CPR certifications are required to be kept up to date
  • currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from the date of hire to schedule for the next available CPR certification class provided by FHCN

Job Qualifications

  • ability to communicate effectively and openly with interdisciplinary team members on patient status and needs
  • ability to meet standardized competencies for the position established by FHCN PACE
  • medically cleared for communicable diseases with up-to-date immunizations
  • valid California driver’s license with an acceptable driving record
  • current certification in CPR, first aid, and defensive driving
  • ability to prepare basic correspondence and reports using Microsoft Word
  • ability to create tables and simple displays in Microsoft Excel
  • ability to create basic presentations in Microsoft PowerPoint

Job Duties

  • effectively communicates with participants, families, staff, and other agencies and departments
  • drives a specially equipped van to deliver physically limited persons to varied destinations
  • assures that wheelchair lifts and tie-down equipment are properly operated and that participants are delivered to appointments in a timely manner
  • follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery route
  • assists participants on and off the van in a safe and courteous manner
  • inspects the van and maintains the vehicle in a clean, orderly, and safe condition
  • assures all necessary program records are maintained according to policy
  • observes each participant for any change in physical, mental, emotional, and social functioning and reports such changes to the licensed nurse
  • completes documentation as required
  • assists in the coordination of 24-hour care delivery with assistance from the FHCN Navigation Center
  • performs other projects and duties as assigned
