Job Requirements

  • Must possess a high school diploma or GED certificate
  • Be at least 21 years old
  • Minimum 1-year experience transporting individuals with special mobility needs
  • Valid driver’s license with an acceptable driving record
  • Medically cleared for communicable diseases
  • All immunizations up to date
  • Pass a criminal background check
  • Ability to prepare basic Microsoft Word documents and use Excel and PowerPoint
  • Defensive driving, first aid, and CPR certifications kept up to date
  • Ability to effectively communicate in writing and verbally
  • Ability to lift and move up to 75 lbs occasionally and up to 50 lbs frequently
  • Ability to stand, bend, walk, and remain alert for extended periods
  • Meet standardized competencies for the position
  • Act within the authorized scope of practice

Job Qualifications

  • High school diploma or GED
  • Minimum 1-year experience transporting individuals with special mobility needs
  • Ability to interact in a friendly and professional manner with participants and their families
  • Effective communication with interdisciplinary team members regarding participant status and needs
  • Ability to act within the scope of his/her authority
  • Meet standardized competencies approved by regulatory agencies
  • Medically cleared for communicable diseases and current immunizations
  • Pass required criminal background checks
  • Basic proficiency in Microsoft Word, Excel, and PowerPoint
  • Valid driver’s license with an acceptable driving record
  • Defensive driving, first aid, and CPR certifications renewed as required
  • Ability to effectively communicate written and verbal information

Job Duties

  • Effectively communicates with participants, families, staff, and other agencies and departments
  • Drive a specially equipped van to deliver physically limited persons to varied destinations, ensuring proper operation of wheelchair lifts and tie-down equipment
  • Follow established schedules, adjust schedules for unanticipated changes, and determine the safest and most efficient pick-up and delivery route
  • Assist participants on and off the van in a safe and courteous manner, handling medical equipment and emergencies
  • Inspect and maintain the vehicle in a clean, orderly, and safe condition, reporting maintenance needs
  • Maintain necessary program records and complete documentation according to policy
  • Observe each participant for any change in physical, mental, emotional, and social functioning and report such changes to the licensed nurse
  • Assist in coordinating 24-hour care delivery with help from the FHCN Navigation Center
  • Perform other projects and duties as assigned
