Job Requirements

  • Valid unencumbered Massachusetts nurse practitioner license
  • At least 3 years of patient care experience
  • Experience with Medicaid preferred
  • Ability to conduct in-home patient assessments
  • Strong interpersonal and communication skills
  • Compassionate and empathetic approach to patient care
  • Familiarity with primary care or geriatric healthcare settings preferred

Job Qualifications

  • Valid Massachusetts nurse practitioner license
  • Minimum of 3 years patient care experience
  • Experience with Medicaid preferred
  • Strong communication skills, both written and oral
  • Empathy, compassion, and friendliness
  • Experience in primary care, adult or geriatric care preferred
  • Skilled in electronic medical records (EMR) systems

Job Duties

  • Visit members within geographic area for in-home health risk assessments
  • Conduct evaluation of past medical history, review of systems, and medication reconciliation
  • Perform vital signs monitoring and comprehensive physical exams
  • Utilize evidence-based practice screenings and point-of-care testing as appropriate
  • Identify and document diagnoses for care management by Porter Care Guides
  • Engage in active medical management to support treatment plans
  • Formulate and maintain lists of current and past medical conditions based on clinical judgment and assessment findings
