Job Requirements
- Valid unencumbered Massachusetts nurse practitioner license
- At least 3 years of patient care experience
- Experience with Medicaid preferred
- Ability to conduct in-home patient assessments
- Strong interpersonal and communication skills
- Compassionate and empathetic approach to patient care
- Familiarity with primary care or geriatric healthcare settings preferred
Job Qualifications

- Minimum of 3 years patient care experience
- Experience with Medicaid preferred
- Strong communication skills, both written and oral
- Empathy, compassion, and friendliness
- Experience in primary care, adult or geriatric care preferred
- Skilled in electronic medical records (EMR) systems
Job Duties
- Visit members within geographic area for in-home health risk assessments
- Conduct evaluation of past medical history, review of systems, and medication reconciliation
- Perform vital signs monitoring and comprehensive physical exams
- Utilize evidence-based practice screenings and point-of-care testing as appropriate
- Identify and document diagnoses for care management by Porter Care Guides
- Engage in active medical management to support treatment plans
- Formulate and maintain lists of current and past medical conditions based on clinical judgment and assessment findings
