Job Requirements
- Active Nurse Practitioner license in Louisiana
- DEA license required
- experience in home health, hospice, or facility-based care
- strong communication skills
- ability to work independently
- familiarity with telehealth and post-acute care environments
- willingness to participate in on-call rotations

Job Duties
- Provide primary medical care services in home, facility, and telehealth settings
- conduct physical exams, wellness visits, and manage chronic and acute conditions
- order and interpret diagnostic tests and labs
- prescribe and manage medications and individualized care plans
- coordinate care with interdisciplinary teams and community partners
- promote preventive care, patient education, and continuity of care
- participate in after-hours or on-call rotation as needed
