Job Requirements

  • Active Nurse Practitioner license in Louisiana
  • DEA license required
  • experience in home health, hospice, or facility-based care
  • strong communication skills
  • ability to work independently
  • familiarity with telehealth and post-acute care environments
  • willingness to participate in on-call rotations

Job Duties

  • Provide primary medical care services in home, facility, and telehealth settings
  • conduct physical exams, wellness visits, and manage chronic and acute conditions
  • order and interpret diagnostic tests and labs
  • prescribe and manage medications and individualized care plans
  • coordinate care with interdisciplinary teams and community partners
  • promote preventive care, patient education, and continuity of care
  • participate in after-hours or on-call rotation as needed
