Job Requirements

  • Minimum of master’s degree in nursing focused on Family, Adult, Geriatric, or Adult-Geriatric Nurse Practitioner
  • 3-5 years Nurse Practitioner experience preferred but new graduates considered
  • Prior healthcare experience such as medical assistant, certified nursing assistant, EMT/paramedic, lab assistant, registered nurse preferred
  • Active, unencumbered Nurse Practitioner license and must reside in state
  • Current board certification with ANCC or AANP
  • BLS and CPR certification required
  • Active, unencumbered DEA licensure or eligibility
  • Valid driver’s license, car insurance, and access to a vehicle
  • Ability to conduct in-home visits and telehealth
  • Ability to lift up to 30 lbs and assist with patient transfers including lifting up to 50 lbs
  • Ability to stand for extended periods
  • Willingness to travel locally up to 100%
  • Proof of auto insurance
  • Ability to use Microsoft Office applications and Electronic Health Records

Job Qualifications

  • Master's degree in nursing focused on Family, Adult, Geriatric, or Adult-Geriatric Nurse Practitioner from an accredited program
  • Board certification by ANCC or AANP
  • Active unencumbered Nurse Practitioner license in the job state
  • BLS certification
  • CPR certification
  • Active unencumbered DEA licensure or eligibility
  • Experience diagnosing and treating chronic and urgent medical/psychological conditions in older adults
  • Proficiency with Microsoft Office, Electronic Health Records, Scheduling Software, and communication platforms
  • Excellent verbal, written, presentation, and interpersonal communication skills
  • Experience in panel management, primary care, geriatrics, internal medicine, palliative care, and end-of-life communication
  • Ability to conduct in-home and telehealth visits
  • Valid driver’s license, auto insurance, and reliable transportation
  • Preferred experience in value-based care, home care, hospice, palliative care, telehealth, and complex patient care

Job Duties

  • Practice the WellBe mission by delivering CompleteCare to patient panels
  • Support in-home, patient-facing visits to meet patient care needs
  • Partner with clinicians to manage the health and well-being of patients in their homes
  • Engage patients into the WellBe program and support ongoing care needs
  • Facilitate decision-making for staff, patients, and families through education
  • Perform primary care services including screenings, examinations, assessments, planning, and documentation
  • Address urgent care needs including transition of care visits
  • Collaborate with community teams to complete patient care assessments and plans
  • Order, interpret, and analyze diagnostic tests, determining follow-up needs
  • Prescribe and manage medications and treatments within state supervisory agreements
  • Update and maintain accurate patient records regularly
  • Deliver care and conduct reviews with supervising physicians per regulations
  • Maintain quality care standards and meet company metrics
  • Manage clinical documentation, coding, HEDIS measures, patient engagement, and medical expenses
  • Serve as a resource for patients with critical conditions by providing counseling and guidance
  • Utilize internal and external resources including behavioral health protocols
  • Participate in interdisciplinary team meetings
  • Maintain active, unencumbered state licensures and national certifications
  • Provide palliative and end-of-life care support
  • Facilitate family meetings to address complex family dynamics and goals of care
  • Adhere to a professional code of conduct and documentation standards
  • Perform additional tasks as needed to meet team goals and objectives
