Job Requirements
- Must have or be eligible to have a current and unrestricted NP or PA license
- Current DEA certificate required prior to start date
- BLS certification
- At least two years of experience practicing as an N.P. or P.A.
- Two years of experience in Primary Care or Adult/Geriatrics
Job Qualifications
- Must have or be eligible to have a current and unrestricted NP or PA license
- Experience with risk adjustments and/or home assessments
- Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and Health Risk Assessments (HRAs)
- Embrace teamwork and the opportunity to collaborate with talented colleagues
- Certificate of completion from an accredited Nurse Practitioner Program or Physician Assistant Program

Job Duties
- Evaluate and treat center patients in accordance with standards of care
- Care coordination with other providers, specialists, and care facilities
- Perform comprehensive in-home assessments for identified patients of the practice and assisting the primary care team in better managing chronic diseases
- Complete physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings
- Communicate assessment findings that will be used to inform the PCP of potential gaps in care
- Will have support of Medical Assistants while seeing patients to allow more time for direct patient care
- Highly relationship oriented and ability to passionately serve the aging and elderly population
- Educate patients on topics such as disease process, medication, and compliance
- Comply with all HIPAA regulations and maintain security of protected health information
