Job Requirements
- Minimum of two to five years experience in relevant clinical area
- Nurse practitioner program completion with board certification
- Active nurse practitioner license
- Active DEA license
- Certification to prescribe medications legally
- Strong communication skills
- Ability to adapt to value-based care model
- Basic computer proficiency
- Willingness to participate in tuberculosis screening program
- Patient-facing capability
Job Qualifications
- Successful completion of nurse practitioner program with board certification
- Minimum of two to five years experience in primary care, geriatrics, complex care, or long-term care
- Active, unrestricted nurse practitioner license in state of practice
- Active and unrestricted DEA license
- Appropriate certification to write prescriptions according to state and federal regulations
- Excellent written and verbal communication skills
- Experience in integrated team-based care
- Basic computer skills including email and EMR
Job Duties
- Evaluate and treat center patients following standards of care
- Monitor medical care quality and outcomes using data and chart reviews
- Coordinate patient services such as specialty referrals and home health care
- Lead daily care team huddles and promote collaboration
- Complete timely and accurate medical documentation
- Participate in local primary care on-call program
- Ensure compliance with licensing and certification requirements
