Job Requirements

  • Minimum of two to five years experience in relevant clinical area
  • Nurse practitioner program completion with board certification
  • Active nurse practitioner license
  • Active DEA license
  • Certification to prescribe medications legally
  • Strong communication skills
  • Ability to adapt to value-based care model
  • Basic computer proficiency
  • Willingness to participate in tuberculosis screening program
  • Patient-facing capability

Job Qualifications

  • Successful completion of nurse practitioner program with board certification
  • Minimum of two to five years experience in primary care, geriatrics, complex care, or long-term care
  • Active, unrestricted nurse practitioner license in state of practice
  • Active and unrestricted DEA license
  • Appropriate certification to write prescriptions according to state and federal regulations
  • Excellent written and verbal communication skills
  • Experience in integrated team-based care
  • Basic computer skills including email and EMR

Job Duties

  • Evaluate and treat center patients following standards of care
  • Monitor medical care quality and outcomes using data and chart reviews
  • Coordinate patient services such as specialty referrals and home health care
  • Lead daily care team huddles and promote collaboration
  • Complete timely and accurate medical documentation
  • Participate in local primary care on-call program
  • Ensure compliance with licensing and certification requirements
