Job Requirements
- valid paramedic license in Ohio
- minimum of two years of experience as a paramedic
- clean driving record
- ability to operate community care vehicle

Job Qualifications

Job Duties
- perform in-home assessments and urgent care interventions
- work in partnership with physicians, nurses, and social workers
- respond to patients flagged through remote monitoring and clinical team referrals
- participate in patient discharge planning and community follow-ups
- operate a fully equipped community vehicle (no ambulance)
- deliver high-quality care under progressive protocols tailored to senior patients
- conduct home safety checks, patient education, and wellness follow-ups
