Job Requirements
- Master's degree in social work
- Current Ohio social work license
- CPR certification
- 1-3 years of job-related experience in a healthcare setting
- Ability to perform physical tasks including standing, stooping, bending, and lifting up to 50 pounds
- Preferred licensure as L.I.S.W. or I.S.W
- Experience with clinical services in home care or community/outpatient settings
- Basic computer skills preferred

- Licensed Independent Social Worker (L.I.S.W) or Independent Social Worker (I.S.W) preferred
- Current Ohio social work license
- CPR certification
- 1-3 years of related healthcare experience
- Experience in clinical and community settings preferred

Job Duties
- Provide social work services to patients as per physician's plan of care
- Conduct assessments of patients' psychosocial needs in home settings
- Develop and implement individualized care plans
- Coordinate with healthcare providers, community resources, and family members
- Advocate for patient needs and facilitate access to social and medical services
- Document all patient interactions and care activities accurately
- Maintain compliance with licensure, certification, and organizational policies
