Job Requirements

  • Master's degree in social work
  • Current Ohio social work license
  • CPR certification
  • 1-3 years of job-related experience in a healthcare setting
  • Ability to perform physical tasks including standing, stooping, bending, and lifting up to 50 pounds
  • Preferred licensure as L.I.S.W. or I.S.W
  • Experience with clinical services in home care or community/outpatient settings
  • Basic computer skills preferred

Job Qualifications

  • Master's degree in social work
  • Licensed Independent Social Worker (L.I.S.W) or Independent Social Worker (I.S.W) preferred
  • Current Ohio social work license
  • CPR certification
  • 1-3 years of related healthcare experience
  • Experience in clinical and community settings preferred
  • Basic computer skills

Job Duties

  • Provide social work services to patients as per physician's plan of care
  • Conduct assessments of patients' psychosocial needs in home settings
  • Develop and implement individualized care plans
  • Coordinate with healthcare providers, community resources, and family members
  • Advocate for patient needs and facilitate access to social and medical services
  • Document all patient interactions and care activities accurately
  • Maintain compliance with licensure, certification, and organizational policies
