Job Requirements

  • Current State Registered Nurse (RN) license
  • minimum of two years' experience with Minimum Data Set (MDS) in a long-term care setting
  • current or eligible for certification in CPR

  • minimum of two years' experience with Minimum Data Set (MDS) in a long-term care setting
  • current or eligible for certification in CPR
  • strong organizational and communication skills
  • ability to collaborate with interdisciplinary teams
  • knowledge of regulatory compliance in skilled nursing care

Job Duties

  • Coordinate care for existing residents and new admissions
  • conduct initial and routine assessments
  • coordinate assessments by interdisciplinary team members
  • ensure compliance and accurate completion of the Minimum Data Set (MDS)
  • oversee and develop the care planning process
  • serve as liaison between community, payer, and corporate office
  • coordinate restorative nursing care program by screening and setting goals for residents
  • educate staff on implementation of care plans
