Job Requirements
- Current State Registered Nurse (RN) license
- minimum of two years' experience with Minimum Data Set (MDS) in a long-term care setting
- current or eligible for certification in CPR

Job Duties
- Coordinate care for existing residents and new admissions
- conduct initial and routine assessments
- coordinate assessments by interdisciplinary team members
- ensure compliance and accurate completion of the Minimum Data Set (MDS)
- oversee and develop the care planning process
- serve as liaison between community, payer, and corporate office
- coordinate restorative nursing care program by screening and setting goals for residents
- educate staff on implementation of care plans
