Job Requirements

  • High school diploma or GED equivalent
  • Experience in long term care preferred
  • Ability to plan and coordinate activities
  • Excellent interpersonal and communication skills
  • Ability to maintain accurate records
  • Ability to work full time including weekends and holidays
  • Proficiency in electronic documentation
  • Compassionate and patient demeanor

Job Qualifications

  • High school diploma or GED equivalent
  • Post secondary education preferred
  • Experience in long term care preferred

Job Duties

  • Plan and coordinate resident activities programs for varying levels of physical and mental abilities
  • Develop rapport with residents and families to understand needs and interests
  • Maintain detailed records including attendance, charts, goals, quarterly notes, hospitalization reviews, discharge summaries, and initial assessments
  • Attend initial and quarterly care conferences with resident care team to determine care plan needs
  • Electronically document Minimum Data Set, RAPS, Triggers, and Care Plan
  • Provide leadership for assigned activities and events
  • Make recommendations to Director of Life Enrichment for alternative programs and changes
  • Promptly answer resident call lights
