Job Requirements
- High school diploma or GED equivalent
- Experience in long term care preferred
- Ability to plan and coordinate activities
- Excellent interpersonal and communication skills
- Ability to maintain accurate records
- Ability to work full time including weekends and holidays
- Proficiency in electronic documentation
- Compassionate and patient demeanor
Job Qualifications
- High school diploma or GED equivalent
- Post secondary education preferred
- Experience in long term care preferred
Job Duties
- Plan and coordinate resident activities programs for varying levels of physical and mental abilities
- Develop rapport with residents and families to understand needs and interests
- Maintain detailed records including attendance, charts, goals, quarterly notes, hospitalization reviews, discharge summaries, and initial assessments
- Attend initial and quarterly care conferences with resident care team to determine care plan needs
- Electronically document Minimum Data Set, RAPS, Triggers, and Care Plan
- Provide leadership for assigned activities and events
- Make recommendations to Director of Life Enrichment for alternative programs and changes
- Promptly answer resident call lights
