Job Requirements
- Licensed vocational nurse license valid in Texas or recognized compact state
- Minimum of 20 hours work per week
- Valid driver's license and insurability through Central Counties Services
- Ability to perform physical tasks including driving, sitting, reaching, stooping, lifting up to 25 pounds
- Ability to work independently and with minimal supervision
- Reliable attendance and punctuality
- Ability to travel frequently within the service area
- Commitment to ethical standards and professional conduct
Job Qualifications
- Must be licensed as a Licensed Vocational Nurse in Texas or a state recognized by Nurse Licensure Compact
- Knowledge of nursing techniques, practices, and procedures
- Skill in problem identification and resolution, effective use of computer and software
- Ability to provide patient medical care, prepare and maintain records, communicate effectively
- Understanding of trauma-informed care and cultural sensitivity
- Ability to plan work and implement directives independently
- Ability to present a professional appearance and conduct
Job Duties
- Responsible for ongoing implementation and monitoring of nursing protocols per guidelines and service contracts
- Administer medications and treatments, monitor vital signs, observe and document patient conditions, assist with daily living activities, provide wound care, collect lab samples, respond to medical emergencies
- Ensure proper medication storage and maintain accurate documentation including nursing notes and patient records
- Collaborate with interdisciplinary teams, provide health education and training to individuals, families and staff, serve as a liaison, participate in team meetings and training
- Conduct focused assessments of client health status and research information for comprehensive nursing assessments
- Ensure compliance with nursing standards and agency policies, practice universal precautions, identify communicable diseases, maintain a safe environment
- Complete all required documentation timely and accurately using Electronic Health Records and Electronic Visit Verification
- Demonstrate high ethical standards, trustworthiness, and maintain positive professional relationships with team members and community stakeholders
- Maintain regular attendance, dependability, and punctuality to ensure consistency and completeness of program processes
- Successfully complete training to maintain competency and cross training to ensure procedural consistency
