Job Duties
- Provide comprehensive house call medical services to homebound patients
- lead a multidisciplinary team of nurses, medical assistants, and therapists
- utilize in-home diagnostic tools such as laboratory services, EKGs, echocardiograms, and ultrasounds
- manage and coordinate specialty care programs including geriatric cardiology, pulmonology, balance, prognostic modeling, and palliative care
- conduct patient assessments to identify and address complex medical and social issues
- maintain accurate patient records and documentation
- participate in ongoing education and credentialing programs to enhance skills in house call medicine
