Job Requirements

  • HUD Service Coordinator experience
  • Two or more years of case management or social work experience
  • Working knowledge of Federal, State, and local elderly and disabled program laws and requirements
  • Must obtain Service Coordinator Certification within 12 months of hiring
  • Proficient with Microsoft Office and Excel
  • Successful completion of background check
  • Successful completion of drug test

Job Qualifications

  • HUD Service Coordinator experience
  • Two or more years of case management or social work experience
  • Working knowledge of Federal, State, and local laws and requirements related to elderly and disabled programs
  • Familiarity with non-governmental networks related to geriatric services
  • Ability to obtain Service Coordinator Certification within 12 months of hiring
  • Proficient with Microsoft Office and Excel
  • Preferred bachelor’s degree in Social Work, Gerontology, Psychology, or Counseling
  • Bilingual in Spanish and English preferred

Job Duties

  • Provide case management and referral services to residents needing assistance
  • Establish contacts with community agencies and service providers
  • Determine best options for service pricing to ensure individualized, flexible, and creative services
  • Advocate for residents to access appropriate resources
  • Maintain accurate documentation and records of services
  • Monitor and evaluate resident progress and service effectiveness
  • Collaborate with property management and external stakeholders to support resident needs
