Job Requirements
- Active RN license in the state of employment or eligible to obtain
- Minimum 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
- Compassionate and patient-centered attitude
- Comfortable providing care in private homes and facilities
- Strong communication skills
- Valid driver's license, auto insurance, and reliable transportation
- CPR certification

- Valid driver's license, auto insurance, and reliable transportation
- CPR certification required
- Preferred experience includes hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac), admissions RN, wound care, float pool, travel nursing, experience with terminally ill or serious illness support
Job Duties
- Be the primary point of contact for patients and families, guiding them through every step of their hospice journey
- Deliver and document skilled, hands-on nursing care based on each patient's individualized Plan of Care in home, assisted living, or other settings
- Perform initial and ongoing assessments to monitor patient condition and adjust care as needed
- Collaborate with interdisciplinary team including physicians, LPNs, CNAs, social workers, chaplains to build and refine personalized care plans
- Provide emotional, physical, and spiritual support to patients and their loved ones including education, comfort, and bereavement guidance
- Teach caregivers how to care for their loved one safely and confidently with practical training and emotional reassurance
- Communicate important updates in patient condition to the care team ensuring timely interventions
- Maintain accurate, up-to-date clinical documentation following infection control and compliance standards
- Support quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care
- Educate patients and families on medication administration, hospice philosophy, symptom and pain management, and end-of-life processes
