Job Requirements

  • Active RN license in the state of employment or eligible to obtain
  • minimum one year RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
  • valid driver's license, auto insurance, and reliable transportation
  • CPR certification required
  • ability to provide compassionate care in diverse settings
  • strong communication and critical thinking skills
  • compliant with state-specific nursing requirements
  • willingness to maintain drug-free workplace adherence

Job Qualifications

  • Active RN license in the state of employment or eligible to obtain
  • minimum one year RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
  • compassionate, empathetic, patient-centered approach with a genuine hospice heart
  • skilled at providing care in diverse settings such as private homes and facilities
  • strong communication and critical thinking skills for emotionally sensitive situations
  • valid driver’s license, auto insurance, and reliable transportation
  • CPR certification
  • preferred experience in hospital settings including Med-Surg, ICU, ER, PACU, telemetry, cardiac
  • experience in admissions, wound care, float pool, or travel nursing
  • knowledge of terminally ill patient care or serious illness support

Job Duties

  • Be the primary point of contact for patients and families, guiding them through their hospice journey
  • deliver and document skilled, hands-on nursing care based on individualized Plans of Care
  • perform initial and ongoing patient assessments to monitor and adjust care
  • collaborate with interdisciplinary teams including physicians, LPNs, CNAs, social workers, and chaplains
  • provide emotional, physical, and spiritual support to patients and loved ones
  • teach caregivers practical care skills and offer emotional reassurance
  • communicate patient condition updates to the care team ensuring timely interventions
  • maintain accurate clinical documentation and follow infection control and compliance standards
  • support quality improvement and scheduling initiatives to ensure compassionate, efficient care
  • educate patients and families about medication administration, hospice philosophy, symptom and pain management, and end-of-life expectations
