Job Requirements

  • Active RN license or eligibility to obtain
  • Minimum one year of RN experience in relevant fields
  • Compassionate and patient-centered approach
  • Ability to provide care in diverse settings
  • Strong communication and critical thinking skills
  • Valid driver’s license and reliable transportation
  • CPR certification

Job Qualifications

  • Active RN license in the state of employment or eligible to obtain
  • One or more years of RN experience in hospice, home health, ICU, oncology, geriatrics, or related fields
  • Compassionate, empathetic, and patient-centered hospice heart
  • Comfortable providing care in diverse settings including private homes and facilities
  • Strong communication and critical thinking skills in emotionally sensitive situations
  • Valid driver’s license, auto insurance, and reliable transportation
  • CPR certification

Job Duties

  • Be the primary point of contact for patients and families guiding them through their hospice journey
  • Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care in various settings
  • Perform initial and ongoing patient assessments to monitor condition and adjust care
  • Collaborate with interdisciplinary team members to develop and refine personalized care plans
  • Provide emotional, physical, and spiritual support to patients and families including education and bereavement guidance
  • Teach caregivers safe and confident care techniques offering practical training and emotional reassurance
  • Communicate patient condition updates to care team to ensure timely interventions
  • Maintain accurate clinical documentation and adhere to infection control and compliance standards
  • Support quality improvement and scheduling initiatives to ensure consistent, compassionate care
  • Assist patients and families in understanding medication administration, hospice philosophy, symptom management, and end-of-life processes
