Job Requirements
- Active RN license or eligibility to obtain
- Minimum one year of RN experience in relevant fields
- Compassionate and patient-centered approach
- Ability to provide care in diverse settings
- Strong communication and critical thinking skills
- Valid driver’s license and reliable transportation
- CPR certification
Job Qualifications
- Active RN license in the state of employment or eligible to obtain
- One or more years of RN experience in hospice, home health, ICU, oncology, geriatrics, or related fields
- Compassionate, empathetic, and patient-centered hospice heart
- Comfortable providing care in diverse settings including private homes and facilities
- Strong communication and critical thinking skills in emotionally sensitive situations
- Valid driver’s license, auto insurance, and reliable transportation
- CPR certification
Job Duties
- Be the primary point of contact for patients and families guiding them through their hospice journey
- Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care in various settings
- Perform initial and ongoing patient assessments to monitor condition and adjust care
- Collaborate with interdisciplinary team members to develop and refine personalized care plans
- Provide emotional, physical, and spiritual support to patients and families including education and bereavement guidance
- Teach caregivers safe and confident care techniques offering practical training and emotional reassurance
- Communicate patient condition updates to care team to ensure timely interventions
- Maintain accurate clinical documentation and adhere to infection control and compliance standards
- Support quality improvement and scheduling initiatives to ensure consistent, compassionate care
- Assist patients and families in understanding medication administration, hospice philosophy, symptom management, and end-of-life processes
