Job Description
Hosparus Health is a compassionate healthcare organization dedicated to providing exceptional hospice and palliative care services to patients facing advanced illness or end-of-life care. Established with a mission to create more meaningful moments for patients and their families, Hosparus Health operates through a holistic, team-based model that supports patients wherever they choose to call home. This includes care provided in private residences, assisted living facilities, and long-term care centers across a range of counties including Butler, Edmonson, Warren, Hart, Barren, Metcalfe, Monroe, Allen, Simpson, and Logan. The organization places a strong emphasis on delivering care that is respectful, personalized, and designed to improve the quality of life for both patients and their loved ones.
Hosparus Health is committed to not just filling roles but fostering a supportive and enriching environment where employees are empowered to exceed expectations in patient care. Employees are encouraged to help cultivate compassionate care practices that ripple through the community, making a tangible difference in the lives of those served. With its premier model of hospice care, Hosparus Health blends clinical excellence with emotional and spiritual support, ensuring that every patient receives care tailored to their unique needs and wishes.
The Certified Nursing Assistant (CNA) role at Hosparus Health is a crucial part of this commitment. CNAs provide essential personal care and homemaker services aimed at promoting safe and clean environments for patients. This role supports hospice patients in diverse settings, including their homes, assisted living, or long-term care facilities, following a specific Care Plan (POC) that meets the standards set out by Hosparus Health. Beyond direct patient care, CNAs at Hosparus Health are involved in shaping the policies and procedures that guide the delivery of hospice services, reflecting the organization's dedication to continuous improvement and holistic care.
The position is full-time with working hours from 8 a.m. to 5 p.m., Monday through Friday, offering a balanced schedule. It requires interaction across multiple counties, necessitating a valid driver's license and auto insurance for travel. The role is designed for compassionate individuals who possess a CNA certification or an active LPN or RN license, with a background in home health aide or nursing assistant experience being preferred. Additionally, proficiency with technology, especially electronic medical records, is strongly favored to support efficient care documentation and communication.
As an employer, Hosparus Health values diversity and inclusivity, providing equal employment opportunities and striving to maintain a workplace free from discrimination and harassment based on race, color, religion, age, sex, national origin, disability, genetics, protected veteran status, sexual orientation, gender identity, or any other legally protected characteristic. Joining Hosparus Health means becoming part of a team dedicated not only to patient care but to enriching the community through compassionate service and support for its most vulnerable members.
