- cna certification required or active lpn or rn license
- valid driver’s license
- auto insurance coverage
- high school diploma preferred
- one year home health aide or nursing assistant experience preferred
- proficient in technology-based tools including electronic medical records

Job Duties
- deliver personal care and minimal homemaker services to hospice patients
- maintain a safe and clean patient environment
- provide services based on CNA plan of care and Hosparus Health policies
- support patients in home, assisted living, or long-term care settings
- participate in developing hospice care policies and procedures
- document care activities accurately
- collaborate with healthcare team members
