Job Requirements
- Must possess a valid driver's license and maintain an acceptable driving record
- must be insurable under professional liability and crime coverage policies
- able to read, write, speak, and understand English
- capable of lifting, transferring, pushing, or pulling up to 50 pounds
- able to sit for up to 8 hours, stand for up to 4 hours, walk for up to 2 hours
- willing to work weekends, evenings, holidays as scheduled
- able to drive up to 6 hours per day
- exposed to infectious diseases and biological hazards
- able to handle the emotional and physical demands of hospice care
Job Qualifications
- High school diploma or equivalent
- certified nursing assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry
- two years experience as a CNA, home health aide, or similar acute care position preferred
- knowledgeable of home health aide practices and procedures preferred
- able to communicate effectively with patients, family, and caregivers
- empathetic and understanding of emotional challenges associated with terminal illness
- capable of maintaining professional boundaries
- skilled in collaborating with interdisciplinary teams
- adheres to hospice care philosophy and standards
Job Duties
- Complete personal care as directed by the hospice nurse
- provide personal care including bathing, oral hygiene, hair care, shaving, skin care, nail care, foot care, and dressing
- offer psychosocial support to patient and family
- assist with meal setup and feeding
- report changes in patient condition to hospice nurse or supervisor
- assist with mobility including walking, transferring, and positioning
- help with toileting needs such as bedpan, urinal, commode, and incontinence care
- assist patients with medication administration and medication reminder systems
- maintain a safe and clean environment
- provide homemaking services and accompany patients to appointments
- perform other care activities including vital signs, catheter care, infection control, and emergency procedures
- deliver and assist with terminal care
- occasionally drive patients to appointments
- maintain comfortable environment adjusting lighting and bed linens
- ensure safety and accident prevention
- provide therapeutic touch
- understand emotional needs of patient and family
- communicate respectfully with sensitivity to cultural and individual needs
- participate in quality improvement projects
- document care and patient observations accurately
- attend interdisciplinary meetings and training
- work weekends, holidays, and on-call shifts as scheduled
- demonstrate initiative and work with minimal supervision
- perform other assigned duties
