Job Requirements
- Certified nursing assistant (CNA) license
- CPR certification
- Current driver’s license and automobile insurance
- Six months of patient care experience in hospice home health ICU geriatric or oncology preferred
- Ability to provide compassionate and empathetic care
- Comfortable providing care in the patient’s home
- Ability to work full-time
- Flexibility and composure while supporting patients and families in crisis
- Willingness to participate in quality improvement activities
- Meet any state-specific regulatory requirements
- Must pass background and drug screening
Job Qualifications
- Certified nursing assistant license
- CPR certification
- Hospice care experience preferred
- Experience in hospice patient care or related fields such as home health or ICU strongly preferred
- Ability to provide compassionate care and support to patients and families
- Comfortable working in patient home environments
- Demonstrated flexibility and composure in crisis situations
- Strong communication and documentation skills
- Commitment to infection control and safety practices
Job Duties
- Report directly to the administrator executive director or patient care manager senior and function under the direction instruction and supervision of the registered nurse and or clinical supervisor
- Help the patient to maintain good personal hygiene
- Assist in maintaining a safe and healthy environment
- Prepare and serve light meals
- Assist in feeding patient when needed
- Assist the patient in ambulation
- Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse
- Assist with light housekeeping tasks such as emptying trash and changing linens
- Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations
- Attempt to promote patient’s mental alertness through involvement in activities of interest
- Give basic emotional and psychological support to the patient and other members of the household
- Report any change in the patient’s mental or physical condition or home situation to the registered nurse and members of the interdisciplinary team
- Demonstrate basic care to the primary caregiver
- Practice infection control measures and standard precautions
- Provide accurate documentation with visit itineraries
- Attend staff meetings
- Participate in at least one quality improvement activity yearly as assigned by supervisor
- Complete documentation in a timely manner as outlined in orientation
