Job Requirements
- Must be able to read, write, speak, and understand English
- possess a valid driver’s license with an acceptable driving record
- insurable under professional liability and crime coverage policies
- physical capability to handle lifting up to 50 pounds
- willing to work weekends, holidays, and evening shifts as scheduled
- able to sit up to 8 hours, stand up to 4 hours, walk up to 2 hours, and drive up to 6 hours per day
- capable of operating transfer devices such as Hoyer lifts
- willing to work in environments with exposure to infectious diseases and bodily fluids
- able to respond to emergency situations
- must demonstrate empathy and maintain professional and personal boundaries
Job Qualifications
- High school diploma or equivalent
- valid Certified Nursing Assistant (CNA) certification in good standing on the Ohio Department of Health Nurse Aide Registry
- minimum two years of experience as a CNA, home health aide or similar role preferred
- knowledge of home health aide practices and procedures preferred
- ability to provide compassionate, patient-centered care
- familiarity with basic infection control and emergency procedures
- excellent communication and interpersonal skills
- understanding of hospice philosophy and end-of-life care dynamics
Job Duties
- Completes personal care as directed by the hospice nurse
- provides personal hygiene assistance including bathing, oral care, hair care, and dressing
- supports patients with mobility and transferring using appropriate devices
- offers psychosocial support to patients and families
- assists with nutritional needs including meal setup and feeding
- reports changes in patient condition to the nursing team
- maintains safe and clean patient environments
- provides homemaking services and accompaniment to appointments
- documents care and patient status in electronic medical records
- participates in interdisciplinary meetings and training
- rotates weekend, holiday, and on-call shifts as scheduled
- assists patients and families with terminal care
- drives patients occasionally to appointments or activities
- adjusts room conditions to maintain patient comfort
- monitors safety to prevent accidents
- applies therapeutic touch techniques upon patient request
- communicates respectfully and sensitively respecting cultural and emotional needs
