Job Requirements
- High school diploma or GED required
- ability to communicate effectively with healthcare professionals and patients
- experience in healthcare coordination or related field
- proficiency in data entry and patient information systems
- ability to explain complex information clearly
- strong organizational skills
- ability to handle sensitive patient data confidentially
Job Qualifications
- High school diploma or GED required
- bachelor's degree preferred
- two to three years of related experience or training strongly preferred
- experience communicating with physicians, nurses, and other healthcare personnel highly preferred
- experience in insurance and Medicare/Medicaid billing and reimbursement preferred
Job Duties
- Attends and participates in staff meetings and community events as requested
- communicates identified needs and solutions to supervisor
- receives and processes referrals from physicians, patients, families, and community
- contacts physicians to secure orders for admission
- coordinates insurance pre-certification authorization
- schedules appointments for informed consents and RN assessment visits
- explains hospice services and benefits to patients and families
- informs team members of appointment times
- completes admission process according to policy
- ensures proper notification of admission to interdisciplinary team and providers
- enters admission data into third-party systems
- ensures patient chart documentation is complete and turns chart over to Team Coordinator
- markets referral sources
- contacts long term care, hospital staff and physicians to promote hospice services
- provides backup support for Hospice Care Consultant
- documents marketing contacts
- performs other duties as assigned
