Job Requirements
- Licensed physical therapist in Washington state or before first day of work
- One to two years of experience as a physical therapist
- Current basic life support (BLS) certification
- Ability to travel to patient homes within South King County
- Reliable transportation and valid driver’s license
- Strong commitment to providing patient-centered care
- Excellent organizational and documentation skills

- Ability to work independently and manage time effectively
Job Duties
- Conduct initial patient assessments and develop individualized treatment plans
- Provide one-on-one physical therapy sessions to patients in their homes
- Monitor patient progress and adjust treatment plans accordingly
- Educate patients and families on exercises and health maintenance techniques
- Collaborate with interdisciplinary teams to coordinate comprehensive care
- Maintain accurate and timely documentation of patient care and progress
- Participate in ongoing training and professional development opportunities
