Job Requirements

  • Bachelor's degree in social work, psychology, human services, or related field
  • Minimum of 2 years experience in case management or family advocacy
  • Experience working with trauma-affected populations
  • Strong organizational and documentation skills
  • Ability to handle sensitive information confidentially
  • Excellent communication and interpersonal skills
  • Ability to work flexible hours as needed
  • Valid driver's license and reliable transportation

  • Knowledge of community resources related to mental health, substance use, and domestic violence
  • Strong interpersonal and communication skills
  • Ability to manage cases and maintain accurate documentation
  • Experience collaborating with law enforcement and community partners
  • Familiarity with trauma-informed care principles
  • Ability to work independently and as part of a multidisciplinary team
  • Commitment to continuous program improvement and community engagement

Job Duties

  • Coordinate the day-to-day operations of the Adverse Childhood Experiences Response Team, including child and family case management and referrals
  • Review police reports related to opioid exposure, domestic violence, family disruption, and other trauma-related incidents
  • Meet with individuals and families to assess needs related to mental health, substance use, and family services
  • Provide guidance on community resources, make referrals, and offer short-term case management in person and by phone
  • Build and maintain strong relationships with community service providers and ACERT partner organizations
  • Collaborate with the part-time ACERT Family Advocate to ensure timely outreach, referrals, and services for referred families
  • Support adult clients with questions related to domestic violence, opioid counseling, and addiction services in the greater Manchester area
  • Maintain accurate and timely documentation of client interactions in the database
  • Manage caseload data and partner with the ACERT Supervisor and project evaluator on reporting and quality improvement efforts
  • Participate in Community Initiatives Department responsibilities, including tabling and community events, as assigned
  • Assist ACERT supervisor in the ACERT Partner Meetings as requested
  • Assist in projects to support ACERT programming as assigned
