Job Requirements

  • Associate degree with minimum two years of case management experience or closely related experience
  • Or high school diploma or equivalent with minimum three years of case management experience or related field experience
  • Valid driver's license
  • Adept with computer programs including Microsoft Office suite and data information management systems

  • Preferred bachelor's degree in human services, behavioral science or related field with one year of case management experience

Job Duties

  • Establish and maintain complete and accurate records of SSVF participants and their progress toward goals and objectives
  • Develop a trusting and helping relationship with homeless veterans without bias regarding their history or circumstance
  • Help SSVF enrolled Veterans connect with available public and private service resources and follow up on referrals to encourage participant progress
  • Coordinate with VA Employment programs and other programs for wrap around support
  • Conduct employability assessments of potential participants including aptitude, readiness for work, and physical capabilities
  • Assist Veterans with creating resumes, employment search plans, job training support and job interview preparation
  • Provide regular feedback on participants' progress and make recommendations for discontinuing or extending services
  • Assist with development and maintenance of a resource base of employers committed to priority hiring of Veterans
  • Coordinate with SSVF Service Coordinator and other staff to ensure wrap around service and communication
  • Participate in in-service training and employment-related events
