Job Requirements
- Associate degree with minimum two years of case management experience or closely related experience
- Or high school diploma or equivalent with minimum three years of case management experience or related field experience
- Valid driver's license
- Adept with computer programs including Microsoft Office suite and data information management systems

- Preferred bachelor's degree in human services, behavioral science or related field with one year of case management experience
Job Duties
- Establish and maintain complete and accurate records of SSVF participants and their progress toward goals and objectives
- Develop a trusting and helping relationship with homeless veterans without bias regarding their history or circumstance
- Help SSVF enrolled Veterans connect with available public and private service resources and follow up on referrals to encourage participant progress
- Coordinate with VA Employment programs and other programs for wrap around support
- Conduct employability assessments of potential participants including aptitude, readiness for work, and physical capabilities
- Assist Veterans with creating resumes, employment search plans, job training support and job interview preparation
- Provide regular feedback on participants' progress and make recommendations for discontinuing or extending services
- Assist with development and maintenance of a resource base of employers committed to priority hiring of Veterans
- Coordinate with SSVF Service Coordinator and other staff to ensure wrap around service and communication
- Participate in in-service training and employment-related events
