Job Requirements

  • Bachelor's degree in social work, gerontology or related field
  • one year of experience in geriatrics, case management, long-term care, home health or medical social work preferred
  • valid driver's license
  • insurance
  • reliable transportation for home visits
  • acceptable criminal records check
  • strong computer skills including basic applications and care management software
  • ability to complete home visits

Job Duties

  • Assess client referrals for program eligibility according to program policy and procedures
  • enroll eligible individuals
  • develop individualized service plans in collaboration with client and/or caregiver that includes community resources as well as ESP services
  • provide information, referrals, and/or resources as needed
  • inform clients of rights and responsibilities
  • monitor service plan costs to ensure adherence to cost caps per procedure
  • secure and monitor services to ensure quality of care supporting health and safety of participants
  • maintain client contact schedule of required interventions via phone or home visits to assess and verify service plans
  • identify and resolve any problems in service delivery
  • monitor changes in client status, conditions, or needs
  • provide support and assistance as needed
  • complete required documentation per procedure
  • maintain ongoing contact and communication with caregivers, family, friends, and informal support sources
  • collaborate with clients, service providers, and others for problem solving and coordination of care
  • complete income and medical expense reviews to determine client co-payments
  • monitor co-payment status
  • manage insurance, Medicare, Medicaid, Advantage plans, referrals to benefit specialists, pursue other funding sources
  • assist with discharge planning upon disenrollment or hospital/nursing home discharge
  • process client appeals and participate in appeal proceedings
  • disenroll ineligible clients per policy
  • cooperate with care management team to ensure client coverage
  • maintain accurate and confidential client records using care management software
  • adhere to confidentiality and HIPAA guidelines
  • follow-up on incidents and health/safety issues
  • complete incident or complaint reports as necessary
  • participate in program quality improvement processes
  • promote a positive community image
  • attend required program and agency trainings and meetings
  • participate in case conferences
  • attend professional meetings, workshops, and seminars
