- demonstrated excellence in patient care and service
- ability to work collaboratively in a multidisciplinary environment
- interest or experience in concierge or personalized medicine models
- fellowship training in internal medicine subspecialties is a plus
Job Duties
- Provide care for a select panel of patients with enhanced access and relationship-based medicine
- deliver care across clinic, home, inpatient, and virtual settings tailored to patient needs
- focus on preventive care, chronic disease management, and urgent care responsiveness
- lead a dedicated care team to ensure communication and prompt resolution of clinical and administrative issues
- collaborate with specialists across over 70 medical and surgical subspecialties
- use advanced tools like Tytocare for remote monitoring and digital assessments
