Job Requirements

  • Current registered nurse license in the state of employment
  • continued employment contingent upon maintaining active license in good standing
  • bachelor of science in nursing preferred
  • five years’ experience in health care, elder care, social work or related industry preferred
  • excellent customer service and conflict resolution skills
  • computer proficiency and ability to document timely and accurately
  • current driver’s license and proof of insurance

Job Duties

  • Understanding the new client including proactively identifying areas of interest, reaching out immediately after the client has been assigned and calendaring a first meeting, conducting in-person nursing initial assessments and reassessments including home safety evaluations, assisting in coordinating all services provided, providing nursing services in accordance with the plan of treatment, ensuring client engagement and visibility, making regular home visits and updating care plans as needed, providing education and instruction to clients, families or staff, reviewing client rates as care progresses, ensuring compliance with state regulations and documentation requirements, providing extra support for new client cases, supporting client retention through hands-on attention and appreciation initiatives, exercising home care knowledge and care management goals, preparing and submitting documentation and clinical notes timely, providing on call and after hours support, performing additional duties as assigned
