Job Requirements

  • High school diploma or GED
  • Current BLS and ACLS certifications
  • Required paramedic licensure within the state of employment
  • Ability to obtain Community Paramedicine certification or complete NEMSA Essentials Course or obtain CP-C licensure within two years of hire
  • Effective communication skills
  • Demonstrated coaching and leadership ability
  • Teaching and educational facilitation skills
  • Familiarity with basic computer functions
  • Team-oriented work style
  • Valid driver's license with an acceptable driving record

Job Qualifications

  • High school diploma or GED
  • Current BLS and ACLS certifications
  • PHTLS and PALS certifications preferred
  • Required licensure to perform as a paramedic within the state of employment
  • Ability to obtain Community Paramedicine certification or complete NEMSA Essentials Course or obtain CP-C licensure within two years of hire
  • Effective verbal, written, and interpersonal communication skills
  • Demonstrated ability to provide effective coaching and leadership
  • Demonstrated teaching and educational facilitation skills
  • Ability to adhere to established standards for educational quality
  • Familiarity with basic computer applications and functions
  • Team-oriented and able to work effectively and efficiently with others
  • Valid driver's license and acceptable driving record

Job Duties

  • Provide high-acuity in-home acute clinical care for conditions such as infections, heart failure, pneumonia, and post-surgical recovery
  • Conduct thorough physical exams and assess the home environment for safety and recovery factors
  • Serve as an on-site extension of the virtual care team with real-time communication for care coordination
  • Respond to crisis situations and rapid response alerts with appropriate interventions
  • Deliver personalized patient and caregiver education on disease management and medication regimens
  • Document assessments, interventions, communications, and outcomes in the electronic health record system
  • Collaborate with physicians, advanced practice providers, case managers, social services, and community partners to ensure continuity of care
  • Participate in research initiatives, program development, mentor new staff, and represent the program in community outreach or quality improvement activities as assigned
